YouWhat is 99284, and why does it say level 4?
Plainly

99284 is a billing code for an emergency department visit. Emergency visits are billed in five levels, by how complex the visit was.

Your records show you were seen in St. Alden's emergency department on March 3 for chest pain. The notes list a chest X-ray, an ECG and blood tests, which are the other lines on this bill.

I can't tell you whether the level or the amounts are right. Questions you could ask the billing office:

  1. How was the visit level chosen?
  2. Has my insurer finished processing this claim?
  3. Can I have an itemized bill?

Answer reviewed against your records and the uploaded bill. Made-up hospital, person and amounts.

Talk your own bill through, next to your own records.

The problem

Bills are written in codes, and the visit was months ago.

A line says 99284. Another says 85025. There's an adjustment, a payment from your insurer and an amount you owe. The visit itself is a blur, and the notes from it live in a different portal, if you can find them at all.

So you either pay it without knowing what it's for, or you spend an afternoon on hold trying to find out.

How it works

Four steps, starting with your records

  1. Start your trial

Start the 7-day free trial and say who the profile is for: you, or someone whose paperwork you help with.

  2. Confirm it's you

Verify your identity and address, choose who can see your profile, sign the consent forms and book an intake visit with a clinician.

  3. Your records come in

Triangle requests your records through the national health-record networks, with a clinician signing the request with you, so the visits behind your bills are on file. Add any records you already have.

  4. Upload the bill, talk it through

Add the bill or explanation of benefits as a PDF or a photo. Ask what a line or term means, what happened at that visit, and what to ask the billing office. The chat answers using your records and what you uploaded.

What Plainly doesn't do

Your own assistant

Ask the assistant you already use

If you use Claude, ChatGPT or Grok, you can connect it to Plainly. With your permission, it reads your records, so it can tell you what happened at the visit on the bill.

Questions to ask it

Once connected, what your assistant reads is handled under that assistant's own privacy policy, which Triangle doesn't control. That includes any bill you paste into it. Connect only assistants you trust. It works only with what's inside Triangle and can't change anything at a hospital, clinic or billing office. Disconnect any time in Triangle under Settings → AI service connections, and access stops immediately.

Your assistant can only read what's in Plainly, so start there.

Questions

Before you upload

What does it cost, and how do I cancel?
Plainly starts with a 7-day free trial. You add a card to start it, and if you cancel before day 7 you're not charged. After that it's $9 a month, and you can cancel any month; your records stay yours. Membership covers Triangle requesting your records from hospitals and clinics and keeping them updated as new ones arrive, chat anytime with the assistant that answers from your own records (which is where you talk a bill through), a review of your medical history by a licensed clinician, connecting your wearables, and uploading anything else you have.
Can Plainly lower my bill or find mistakes in it?
No. Plainly doesn't audit, negotiate or dispute bills. It reads the documents you upload and talks them through with you, next to your records, so you know what you're looking at and what to ask.
Will it have the records from the place that billed me?
Not always. Triangle requests records through Carequality and TEFCA, the national health-record networks, and not every hospital or clinic can be reached that way. Plainly shows what may be missing and helps you request it, and you can upload visit summaries or records you already have.
What will setup ask of me?
What Triangle needs before it can request records in your name: your details and an identity check, confirming your address, choosing who can see your profile and signing the consent forms. You also book an intake visit with a clinician.
What can I upload?
Bills and explanations of benefits as PDFs or photos. For your medical records, PDFs, photos of paper records, discs and faxes, alongside the records requested for you through the national networks.
Will it pick an insurance plan, or tell me what's wrong with me?
No. Plainly doesn't recommend insurance plans, and it doesn't diagnose, treat or prescribe. It helps you understand your records and prepare questions, and it never replaces your care team.
Who sees my bills, and who makes Plainly?
Only you and the people you choose to share your profile with. Your data is never sold or shared for advertising. Plainly is made by Triangle, and your account is with Triangle at app.trianglehealth.com, under Triangle's privacy policy and terms.
